TACKLE & FLAG FOOTBALL - Registration Forms Required

The following forms are physically required in order for Tackle Football Players and Flag Football players to participate on the first day of practice.

Please print and sign, and remit to the league. In this packet, we will need:
- Legible Copy of Birth Certificate
- Image Release Waiver
- Concussion Waiver & Release of Liability
- Medical Clearance Form - send this to your pediatrician and have them sign and date it
- Code of Conduct Form

Forms:
2025 Medical Clearance Form - Please send this to your pediatrician. They must sign and date in 2025 to be valid.

Concussion & Liability Form - Please print, sign, and remit this form to us on Equipment Handout Day.

Code of Conduct Form - Please print, sign, and remit this form to us on Equipment Handout Day.

Social Media Waiver - Please print, sign, and remit this form to us on Equipment Handout Day.

Birth Certificate - Please bring a legible copy of your player's birth certificate with you on Equipment Handout Day.

All of these forms are REQUIRED to take the field.

CHEERLEADING - REGISTRATION FORMS REQUIRED

As part of the AYC and NHYFSC for Cheerleading, they require the following documentation.

All Cheerleaders must be registered on the NHYFSC - Keene Knights website. This is MANDATORY and in addition to our regular Keene Knights registration. This registration is for informational purposes only, generating a player packet - there is no fee.

Your athlete WILL NOT be able to participate without this registration and corresponding packet being signed and turned in to Keene Knights.

ADDITIONAL FORMS NEEDED:
Please print and sign, and remit to the league. In this packet, we will need:

Birth Certificate - Please bring a legible copy of your player's birth certificate with you on Equipment Handout Day.

Code of Conduct Form - Please print, sign, and remit this form to us on Equipment Handout Day.

Medical Release Form - Please print and have your pediatrician complete. This form needs to be turned in by 8/1/25.

Hydration Information - Please review!

All of these forms are REQUIRED to take the field. You child will not be able to join in on practice on day 1 if we do not have everything in hand.
